Don’t see your insurance company?
- You will be provided with an itemized receipt at the end of your visit which contains all of the information your insurer will need to process your claim. Just fill out the personal information requested on the claim form, which your insurance company provides to you, and mail the form along with the insurance copy of the itemized receipt.
Don’t have insurance?
- Cash pricing is available. Payment for services is expected at the time of the service
